Middle School Sports Update: 

Middle school Fall sports are scheduled to begin on September 5, 2024. Tryouts will be held for football, volleyball, boys soccer, and girls track. Sixth grade will not be eligible for football but may try out in the other sports being offered. All students must have their information up to date in Final Forms, have a current sports physical, and attend the virtual parent meeting in order to be eligible to play. As we prepare for the start of team competition again, it is important that students and parents read the following eligibility requirements carefully.

Academic Eligibility: 

• A student must be in attendance 85% for the previous semester. (Cannot miss more than 13 days out of the 90-day semester)

  • This includes all absences (Excused and Unexcused)

• Students may not fail more than 1 core class (Math, English, Science, Social Studies) or more than one Core class and one Elective Class.

• CCS Rule: Students must have a 70% cumulative average for Semester One (August 2020-December 2020).

• A student is ineligible to participate at the middle school level if he/she becomes 15 years of age on or before August 31.

Sports Physical Requirements:

• Students must have a sports physical completed on the North Carolina High School Athletic Association Sports Preparticipation Form, which can be found on the CCS website or by clicking the link.

• Any physicals dated within the year of the entire playing season of your child's chosen sport are acceptable. If the physical date expires during the season, then a new physical must be completed in order to participate in the upcoming season(s) and stay in compliance.

Final Forms is New to CCS!

Final Forms is the new student-athlete registration system that is being utilized by Cumberland County Schools. It is an online enrollment platform that will streamline your child’s information and registration details all in one centralized system. All student-athletes will need to set up a new account for the 2024-2025 school year. Email Mrs. Mack-Malloy if you need any assistance accessing your account or have additional questions.

New Accounts:
Parent Registration Must be completed first.

• Locate the Parent Icon: Find and click on the parent icon on the website.

• Click NEW ACCOUNT: Look for and select the option to create a new account.

• Enter Details:

  • NAME: Type your full name.

  • DATE OF BIRTH: Enter your date of birth.

  • EMAIL: Provide your email address.

• Click REGISTER: Submit your information by clicking the register button.

• Check Your Email:

  • Look for an email from FinalForms within 2 minutes.

  • If you don’t receive it, check your spam or junk folder.

  • If you still can’t find the email, email support to report the issue.

• Open the ACCOUNT CONFIRMATION EMAIL:

  • Find the email from FinalForms Mailman.

  • Open the email and click on CONFIRM YOUR ACCOUNT.

• Create Your Password: Follow the instructions to create a new password for your FinalForms account.

• Click CONFIRM ACCOUNT: Finalize your account setup by confirming.

• Register Your Student:

  • Click REGISTER STUDENT to start the registration process for your first child.

Once a parent has completed their account set up then you may complete the Student Registration:

Click LOG-IN under the parent account. Locate and click the ADD STUDENT button. Type in the LEGAL NAME and other required information. Then, click CREATE STUDENT. If your student plans to participate in a sport, activity, or club, then click the checkbox for each. Then, click UPDATE after making your selection. Selections may be changed until the registration deadline. Complete each form and sign your full name (i.e. ‘Jonathan Smith’) in the parent signature field on each page. After signing each, click SUBMIT FORM and move on to the next form. When all forms are complete, you will see a ‘Forms Finished’ message.

• Important: If required by your district, an email will automatically be sent to the email address that you provided for your student that will prompt form your student to sign the required forms.

• How do I register additional students?

  1. Click MY STUDENTS. Then, repeat steps C through G for each additional student. 

  2. Finally, Mrs. Mack-Malloy will approve each sheet.
